Our office has pulled together the questions on the pre-registration form in a printable format to assist any community organizations or neighbors who would like to help anyone pre-register who may not have access to the website or call center.

Bear in mind these forms are not issued by DC Health and are only intended for volunteers to use in assisting neighbors with getting pre-registered. The forms use the same language provided by DC Health as of March 10, and were put together in partnership with Ward 6 Mutual Aid.
